What is a certification of insurance, aka, a COI?
A certificate of insurance (COI) is a document from an insurer to show that Huntington Beach Union High School District is insured.  They're also known as certificates of liability insurance or proof of insurance. 
 
When we are utilizing another company's facilities, they will sometimes want to see proof of our coverage.  Occasionally they will even want to be listed as an additional insured on our insurance.
 
 
How do I request a COI?
In order to make sure we get the certificate of insurance produced correctly, it is important to fill out the below request form and provide any contracts, facilities use agreements, or any other documents that state the insurance requirements.  These documents will also be your guide to what to fill out on the form below.
