HBUHSD provides dental and vision insurance to all benefit-eligible employees and their eligible dependents. For dental and vision, dependents are covered up to age 19 (or up to age 25 if enrolled as a full-time student).
Vision Service Plan (VSP) offers a network of pre-selected doctors available for your use. If you use the network, you will receive the highest level of benefits the plan offers. While the network is available, you are not required to use it. You always have complete freedom to select any provider each time you need care. The out-of-network benefits, however, are lower and your out-of-pocket costs are higher.

The Delta Dental PPO dentists offer lower negotiated fees with Delta Dental. This means there is a network of participating dentists available for you to use for your dental care that will allow you to pay less out-of-pocket for the services rendered. The Delta Dental PPO program allows you the freedom to visit any licensed dentist, including a dentist from the Delta Dental Premier Indemnity network. However, benefits under the plan are highest if you receive care from a Delta Dental PPO network dentist instead of a Premier or non-Delta Dental dentist.
Under the Delta Dental PPO plan, Delta Dental pays 80% of the allowed fees for covered diagnostic, preventive, basic, cast, and crown benefits during the first year you are eligible. This percentage will increase 10% each year (to a maximum of 100%) for each enrollee, provided that person visits the dentist at least once during the year. If an enrollee does not use the plan during a calendar year, the percentage remains at the level reached the previous year. If an enrollee becomes ineligible for benefits and later regains eligibility, the percentage will drop back to 80%.
