Adding Dependents - 30 Day Rule

Contact the HBUHSD Insurance Department at [email protected] within 30 days of the following qualifying life events:
  • Marriage
  • Change in domestic status
  • New baby
  • New adoption or guardianship 
 
You have 30 days from the date of the qualifying event to enroll the new dependent on your insurance benefits. If you enroll more than 30 days after the qualifying event, your new dependent will not be added to any of our health and welfare benefits until the next open enrollment period (typically October) becoming effective January 1.
 
You must provide corresponding documentation before any changes can become effective (i.e., marriage certificate, certificate of domestic partnership, birth certificate, adoption papers, etc.).
 
 

To add your newborn/adopted child/ward to your insurance plans:

  • Please fill out the paperwork below and return it with a copy of the hospital memo to the HBUHSD Insurance Department within 30 days of the date of birth.
  • Pages 2-5 MUST be filled out and/or signed. The rest is only applicable if you need to make changes to your life insurance coverage or beneficiaries.
  • We will need your newborn's SSN and official birth certificate once you have obtained them.

To add your spouse/domestic partner to your insurance plans:

  • Please fill out the paperwork below and return it to the HBUHSD Insurance Department within 30 days of the event date.
  • Pages 2-5 MUST be filled out and/or signed. The rest is only applicable if you need to make changes to your life insurance coverage or beneficiaries.
  • We will need a copy of your official marriage certificate once it is obtained.
